Registration and Other Information
NEED HELP? Give us a call, text, Facebook message or email with the Package you are wanting and we will send you everything you need to sign up. We will walk you through all steps.
By registering you agree to these terms and paying each cycle.
REGISTRATION: Locks in your spot and price for the entire year!
-When you register, make sure you write your email address, name and child's name in the message area. You can send your payment through your banking account through Zelle at 843-860-9037. Zelle is our preferred way since it does not take a percentage of the payment. We also have Venmo @Ashleigh-Morris-32.
-After you register, forms will be emailed to you.
-After you register, you may go ahead and make the 4 week cycle payment.
Make sure your correct email address is used. If you do not receive an email, contact us or download them from All Forms.

4 ways to fill out Registration forms:
1.) Click the link to the forms below.
2.) Fill out by hand and click on the attachment below: Download, print, scan, and email.
3.) Fill out online: Download All Forms and go to Formswift.com then go to Edit PDFs Online and upload this PDF, fill out the form and send. (This form may be time consuming. You can sign up on Formswift.com for free to save your work).
4.) Call Discipled Youth Academy and speak to a representative.

CLOSURES: DYA does not prorate payments. Make-up days will be scheduled or an online classroom will take place for closures for weather, Acts of God or other.
ABSENCES: Whether or not your child attends class during all days of the cycle, you are responsible for payment in full.
PAYMENT METHODS: Payments may be made on PayPal to discipledyouthacademy@gmail.com or in person by check or money order. A new payment method is coming! Payments will be due one week before the new four week cycle starts. Payments are due on that Tuesday by 3:00pm.

- Pay less. For those choosing the Two Day Option and using Zelle. We take off $150 for those paying for the year in full for the 2 day a week package! Make your payment with Zelle through your banking.
- A material list is provided in the classes tab after clicking on either Little Shepherds or Ambassadors. Read each class carefully to make sure you have all the curriculum books and materials necessary.
- Each student will need to bring a packed lunch. Microwaves will not be available.
- We are not a peanut or other allergy free facility. If your child has dietary needs, please contact us directly.
